Pricing in private hospitals needs to be monitored: NIC chief

Public sector insurer, National Insurance Company (NIC) on Wednesday made a strong pitch for monitoring pricing in private hospitals and a need for a regulator in this regard.

According to AV Girija Kumar, Chairman and Managing Director, NIC, there was a need to have a regulator who would monitor pricing in private hospitals and stop them from charging exorbitant rates.

“There is a need for monitoring pricing in private hospitals,” he told newspersons on the sidelines of an interactive session organised by the MCC Chamber of Commerce & Industry.

“Healthcare financing and delivery should be integrated and this will help the insured people immensely,” he added.

Tamilnadu mulls accreditation for hospitals offering health insurance

Tamil Nadu plans to get hospitals offering healthcare services under the State-sponsored health insurance accredited for quality. The move is expected to help healthcare providers adopt standard operating procedures and free patients from needless expenditure necessitated by infection, wrong diagnosis and surgical errors.

The National Accreditation Board For Hospitals and Healthcare Providers will look at 149 objective parameters for entry-level acceptance for small hospitals. For hospitals with bed strength above 50, the board has set over 600 standards. Infection control, pharmacy management, care of patients and patient rights are among key benchmarks. Health centres need pay Rs.10,000 as certification fees, and State health officials are keen to get a good portion of the 834 government and private hospitals offering the insurance scheme.

J Radhakrishnan, Tamil Nadu Health Secretary, said smaller health centres should play a proactive role in getting their facilities accredited so that the scheme could expand further.

C Vijayabaskar, Health Minister, Tamil Nadu, said a meeting to discuss modalities of getting these hospitals accredited and estimate the associated expenditure will take place soon.

He added that the State health scheme has touched over seven lakh lives so far, spending close to Rs.1,509 crore since its inception in January 2012. Over 2.74 lakh patients have received life-saving surgeries at Government hospitals. The scheme has expanded its coverage to 1,016 diseases now, and is looking at including heart transplants.

Improved quality reduces large needless costs for all stakeholders – the insurer, the hospital and the State, said Somil Nagpal, a World Bank health specialist analysing Government health schemes in the country. He co-authored a report “Government-sponsored Health Insurance in India-Are you Covered?” It concluded saying a fourth of India’s population by 2010 was benefited by different ways from Government health schemes.

“Costs of excessive healthcare due to wrong diagnoses and mistakes during surgeries touched $41.5 billion in 2006, a number equal to total healthcare expenditure in India during the year,” he said, adding that the NABH has only 250 hospitals accredited in a country of over one lakh health centres.

NABH sets new entry-level standards for Accreditation of Hospitals

The National Accreditation Board for Hospitals and Healthcare Providers (NABH) has released a new batch of entry-level standards for accreditation of smaller hospitals. As the name suggests, Entry Level Standards are meant for hospitals who want to get started on the quality certification journey, but are unable to do so due to the stringent requirements of the full NABH accreditation.

“The whole idea behind introducing this new set of standards is to become more inclusive; to get a number of hospitals to join the quality journey. With the full set of NABH standards, recognised by the International Society for Quality in Healthcare [ISQua] many smaller hospitals cannot even hope to apply for,” explains K.K.Kalra, CEO, NABH. Currently, there are only 227 hospitals with NABH accreditation, and about 1200 in various stages of application.

The new standards will be a foot in the door for a number of small hospitals who find the rigor of the NABH full standards beyond their capacity. At the same time, Dr. Kalra hastens to reassure those in the field that there will be no dilution of standards whatsoever.

“There are two types of new standards: One, for hospitals with over 50 beds, and two, for small hospitals with less than 50 beds. While the full NABH has 102 Standards and 636 Objective Elements, there are about 45 Standards and 173 Objective Elements for 50-bedded hospitals and 41 Standards and 149 Objective Elements for less than 50 beds,” he adds.

It is by no means easy to get accreditation even at the entry level without adhering to a set of regulations, Dr. Kalra says, and without hard work on the part of the hospital, it will not be possible.

The NABH standards were put in place about seven years ago to provide quality assessments for hospitals in the country. The NABH accreditation has been viewed as the ideal qualifying criteria for selection or empanelment by the Ministry of Tourism, Central Government Health Services, several public and private insurance companies.

In fact, a number of insurance companies are considering making the entry level standards the bare minimum for empanelment of hospitals, Dr. Kalra added. Once the smaller hospitals get in the race, they can even work on moving to full NABH accreditation. “But this is the entry point. Small hospitals in small cities too have a right to joining the quality race. Why should they be kept out?”

Draft Minimum Standards for Various Categories of Clinical Establishments

The Clinical Establishments (Registration and Regulation) Act, 2010 has been enacted by the Central Government to provide for registration and regulation of all clinical establishments in the country with a view to prescribing the minimum standards of facilities and services provided by them. The act has already taken effect in 4 States and all Union Territories since 1st March 2012 vide Gazette Notification dated 28th February 2012.

Keeping in view to implement the act across the country the National Council for Clinical Establishments under the Chairmanship of Director General of Health Services, Government of India in consultation with various stakeholders has prepared draft minimum standards for various categories of Clinical Establishments for implementation of the Clinical Establishments Act.

The comments, suggestions, objections, including deletions /additions if required in the draft documents are invited from public at large, including the stakeholders like hospitals and other clinical establishments, consumer groups etc.
