NABH endorses DMAI’s ‘Patient Charter’ on rights & responsibilities of patients

Understanding the need to protect the interest of the patients across the country, the National Board for Accreditation of Healthcare (NABH) recently endorsed and supported a unique programme called ‘Patient Charter’ initiated by the Disease Management Association of India (DMAI). This patient-centric programme is a first of its kind initiative propagated to facilitate the needs of the patients so as to keep them and their family informed about their rights and responsibilities while using healthcare facilities.

The Charter was drafted by DMAI in consultation with experts from the healthcare community making India the only country in Asia Pacific to have such a detailed yet crisp patient charter. Under the said charter the doctors and other healthcare providers are expected to display the charter in their reception area prominently in local, Hindi and English languages.

The Charter will basically work as a guideline for the healthcare providers to ensure that the patients are duly updated on all the healthcare needs. Most importantly it stresses on the need for adopting patient friendly environment through documented procedure for obtaining patient or family’s consent for informed decision making while availing healthcare needs so as to involve the patient and family in decision making processes.

According to Rajendra Pratap Gupta, president DMAI, even though healthcare is all about helping the patients they are seldom taken into consideration in any decision making process. He insisted that it is high time for all to come together and work for the upliftment of the patient community so that they are also given a fair chance to express their concerns, issues and doubts to the concerned authority.

“Patient Charter is one such initiative that is not just a document that outlines the doctor’s role, responsibilities and code of practice but also the patient’s rights and his duties. Most of the medical failures happen due to either ignorance of patients or negligence of practitioners or providers of healthcare and we wish to caution both the patient and the providers on the seriousness and the outcomes of not working together,” Gupta stated.

He further stressed that communication should become the first line of treatment’ and that patient charter will play a significant role in empowering the patient through this initiative. Agreeing with him Dr Gayatri Vyas Mahendroo, director, NABH said that the it is important to educate patient and family members about the basic healthcare proceedings and risks so enable them to make informed decisions and involve them in the care planning and delivery process.

“Patient and families have a right to information and education about their healthcare needs and on the expected costs. Thus to empower the consumer on these issues, NABH along with DMAI fully support the patient charter and believe that it should be promoted with all healthcare providers in India, be it individual doctors or small clinics or large hospital chains, on the lines of ‘Jago Grahak Jago’ campaign,” Mahendroo insisted.

QCI urges hospital managements not to compromise with patients safety

Quality Council of India (QCI) has urged all the healthcare providers in the country to ensure that they follow all quality standard guidelines recommended by the government to avoid any complications that may compromise patients safety in the hospitals. This demand comes in the wake of the devastating fire at Amri Hospital that claimed lives of 190 people due negligence on the part of the hospital.

With a view to ensure that all the hospitals in the country are sensitised about the importance of following all the quality and safety regulations, QCI had recently organised its first national quality conclave for the healthcare sector in Bangalore. The conclave focused on enhancing the quality in healthcare system of the country and deliberate on the issues that hinder in deliverance of quality service.

According to Rajendra Pratap Gupta, member, Healthcare-Quality Council of India, “It is not possible to provide accreditation to all the hospitals at once since its takes a lot of time as well as man power to do that. However it does not mean that the hospitals that are not under National Accreditation Board for Hospitals and Healthcare Providers (NABH) should fall behind. We want the healthcare providers to understand that accreditation is important but at the same time it is not beyond quality. We want to ensure and change the way things are perceived today by stressing on keeping up with our motto to ensure quality healthcare in the country.”

He informed that as on date NABH, which is a constituent board of QCI has accredited nearly 140 hospitals and another 500 plus hospitals are preparing for accreditation in the country. The accreditation programme by NABH covers all the facets of patient safety including surgical safety, medication management, hospital infection and patient communication. The primary goal of accreditation is to ensure that the hospitals not only perform evidence based practices but also give importance to access, affordability, efficiency, quality and effectiveness of healthcare. However, he stressed that though approval is essential steps must be taken to look beyond accreditation to address and deal with quality issues in the present healthcare system.

The conclave that was co-organised by Bronze Certification of Lean by Simpler UK at M S Ramaiah Medical College & Hospital in Bangalore provided critical inputs pertaining to various facets of quality in healthcare.

“Quality in healthcare is of paramount importance as it directly impacts human lives and unlike quality in other sectors, quality in healthcare relates to a hospital monitoring and measuring clinical outcome on a continuous basis. Through this conclave, which will be a quarterly event, we want to bring in all the healthcare providers under the same roof so that we can exchange new ideas and deliberate on issues and challenges that will have a far reaching effect on the way healthcare system is dealt in the country,” Gupta said.

He further informed that looking at the quality issues in the East zone, QCI has decided to organise the second conclave in Kolkata in March next year followed by west and north zones respectively.

Hospital-acquired infection norms to be made stringent in India

For about 500 hospital applicants seeking accreditation from the National Accreditation Board for Hospitals and Healthcare Providers (NABH), they would have to comply with additional guidelines on controlling hospital-acquired infections.

The NABH has formalised an agreement with medical-devices company Becton Dickinson (BD) to tackle hospital infections, even as it processes two more tie-ups with healthcare companies Johnson and Johnson and 3M, Dr Giridhar J. Gyani, NABH Chief Executive and Secretary-General of Quality Council of India (QCI), told Business Line.

The tie-ups

The J&J tie-up is to train new surgeons, before they get into their jobs, on aspects including controlling infection, he said. The 3M tie-up is to tackle medication errors, he added. A quasi-government organisation, the NABH is part of QCI, and covers healthcare organisations.

With the World Health Organisation pegging the rate of hospital acquired infections at between 15 and 40 per cent in developing countries, Dr Gyani said hospitals needed to be educated on such infections. Over 100 hospitals in the country are NABH accredited, he said, and it is expected that they keep a check on infections in hospitals.

NABH guidelines

Though the guidelines are voluntary, it will be applicable for the 500 applicants seeking accreditation, he added. The guidelines include features such as safe injection practices, waste management and infusion safety, and optimising/ minimising the stay of patients in hospitals, so that it benefits and is economical to both hospital and patient, he said.

The country is estimated to have about 40,000 healthcare organisations of all hues, and about half this number is institutions with over 20 beds, he said.

Private alliance

Responding to whether healthcare organisations would trust guidelines evolved with other private companies (BD, J&J and 3M, in this case), Dr Gyani said, the alliance with firms like BD and J&J were because they have large workforces for training. Their guidelines would be aligned with NABH, and “we will keep an eye that they do not promote their own products in the training programmes,” he said.

Emphasising that the process was above board, he said the guidelines took about six months to design, and they focus on the “national cause and not a company’s cause”.

The Memorandum of Understanding signed with BD is for two years and similar agreements will be signed with the other two companies, he said.

The NABH’s efforts to tackle the problem of hospital-acquired infections assumes significance given the prevalence of the problem, besides emerging concern over drug-resistant “super-bug” strains of bacteria originating from the country.
